Once you have paid and booked your session, you will be emailed an online  pre-questionnaire, to be filled out and to be returned to us 72 hours before the scheduled appointment. Please check your spam if you have not received it within 10 minutes of your booking. It will come by the email: clinic@chakaura.com There is a section in the online questionnaire included to ask us those special questions you might have before the meeting. IMPORTANT INFORMATION:  **See Confirmation Email following payment for Further Instructions and Link to Form** Missed appointments are generally not refunded or rescheduled unless unusual circumstances are shown with proof of incident.  Please check our availability before deciding to re-schedule. On-Line Live/sessions are recorded for quality and service optimization. If, for some reason, we cannot make the appointment due to outstanding complications or health reasons, we will reschedule during business hours at your convenience within an acceptable time. If this does not work then we will refund you your investment in yourself.
